Please fill out this form accurately using the provided Rating Key below. This form is used as a tool to gauge a Leader or Member’s performance while serving in the WisCorps Crew Program. The completed form will be saved in the file of that participant and will be available to WCC staff for review. This form can also be used as a conversation piece during 1 on 1 conversations with service people about their performance. Crew Program Coordinators will fill out this form to evaluate Crew Leaders. Crew Leaders will fill out this form to evaluate Crew Members. The Crew Program Manager will complete this form if a Coordinator or Crew Leader is unavailable. Please be thorough in answering all questions. Rating Key: 0 = not applicable 1 = needs improvement 2 = fair 3 = meets expectations 4 = exceeds expectations 5 = significantly exceeds expectations
